Step 1: Go to travel.state.gov (http://travel.state.gov)

Step 2: Save time and fill out the (DS 11) Form online. Wait to sign until you apply in person

Step 3: Print the form

Step 4: Collect your citizenship evidence ( i.e. birth certificate, naturalization certificate, consular report of birth aboard,

Step 5: Make a front and back photocopy of your government-issued ID ( such as: Driver License )

Step 6: Take a passport photo
REQUIREMENTS:

• must be in color
• must be 2’ x 2” inches in size
• Taken within the last 6 months
• Plain white background
• Must remove eyeglasses

  Step 7: Bring money for passport fees ( common forms of payments are accepted: Cash, Credit/Debit Card, Check/Money orders

  
  

  Step 8: Go to travel.state.gov (http://travel.state.gov) to find your local acceptance facility

  
  

  Step 9: Visit your local acceptance facility ( give all the necessary papers/payments/take oath)
